Information and Assistance

Knowledgeable staff from Aging and Adult Services is available to provide information and answer questions about the programs and services available through this office and help you to understand the eligibility requirements for each of the programs and services. The staff also provides assistance with benefit applications such as PACE, Property Tax/Rent Rebate, City garbage fee exoneration and Township water and sewer rebates. These forms can be obtained by contacting the Office of Aging and Adult Services. Information and listings of subsidized apartment buildings, Home Health Care Agencies, Assisted Living Facilities, Personal Care Homes, Nursing HomesContinuing Care Retirement Communities and health insurance counseling are also available. You may also access the Benefits and Rights booklet for additional information on programs available for older Pennsylvanians.

Consumers and family members interested in applying for the Aging Waiver Program or Nursing Home Transition (NHT) can contact the Independent Enrollment Broker, Maximus at 1-877-550-4227.

Community Health Choices (CHC) will begin in Lehigh and Northampton Counties in January 2020. CHC is a mandatory managed care program for Pennsylvanians who are dually eligible for both Medicare and Medicaid (Medical Assistance), enrolled in Medicaid Waivers for physical disabilities, and Medicaid-eligible consumers who reside in a nursing facility. As a managed care program, CHC give individuals the choice of health plans to best match all of their health care and long-term care needs.
